Zayan LLC ActiveCare Toxicology Program
Zayan LLC ActiveCare Toxicology Program is dedicated to providing the highest-level drug testing services for the Illinois Department of Children and Family Services throughout Illinois. Our staff is knowledgeable regarding all collection procedures and very detailed, ensuring you unparalleled excellence.

Donor Services: At your appointment, you will be asked to provide a valid photo ID and specimen based upon the criteria elected by DCFS. All collections are observed by our trained staff, and you will be given a paper receipt documenting your visit. Your specimen will go out same day via FedEx to Abbott Laboratory. If you have questions regarding lab tests ordered or results, please contact your caseworker/supervisor.
A valid photo ID must be presented at every scheduled appointment. Valid Identification accepted includes:
- Driver’s License
- Passport
- State Issued ID
- FOID Card
- ID issued by a federal, state or government agency
Referral Process for Caseworkers:
ALL client referrals must be entered and approved by the supervisor through SACWIS- 24 hours prior to appointment date.
Include appointment time and any important details needed for that specific client.
No Same Day Referrals will be accepted; Emergency referrals by request.
*For Zayan LLC ActiveCare Mobile Referrals, all referrals are pulled at 5pm for the next day*
Private agencies can utilize the same schedule/days/ hours and SACWIS referral process.
